Prior to scheduling appointment please verify the following:
You are 18 years of age or older. Ages 18 - 25 require 2 certifications with one being from an MD or DO. We will be happy to make a referral if needed.
You have a valid unexpired Mississippi driver’s license or id.
You have medical records that contain your qualifying medical condition.
